In my experience, tricyclic antidepressants, followed by Gabapentin or Pregabalin are the most beneficial. Although supporting evidence is scant, pramipexole, Clonazepam and alpha-lipoic acid can be tried if the other medications fail. Cognitive behavioral therapy has also been tried with varying success
