Depends: On the organ and diagnosis. If it is for liver cancer, then usually a percutaneous (through the skin) liver biopsy is done (though not the only route) under direct visualization into the tumor.

It depends: The type of biopsy recommended depends on many things such as location and type of mass that is being biopsied. Some are done with a fine needle, some require more extensive incisions. Your doctor will explain exaclty what type of biopsy it will be.
