No way: At 16 weeks there is no way an intrauterine fetus wouldn't be seen on ultrasound. Also, home pregnancy test is quite sensitive.

Unlikely: To be sure, repeat the urine pregnancy test now. The sensitivity of the Urine BHcg test for pregnancy is exceptionally good these days, but could miss an very early pregnancy one week after intercourse. The only time I have seen it be inaccurate as compared to the blood test was a patient who bought it at the dollar store.
