Must be checked: At age 18 its unlikely you have coronary disease but this neds to be checked this could be multiple issues ranging from reflux to mitral prolapse . I would see you primary if you don't have one see an ER they can do some simple tests and find out the cause of these severe pains.

Coronary involvement: Given your age this is unlikely to be coronary artery disease, but that is the biggest concern at this point, particularly if you have risk factors. Other problems to consider include vasospasm of the coronary arteries, or a vasculitis which, though quite rare, can be a serious condition. This could also be plain old panic attack masquerading as heart or lung problems.
