Many possibilities: Shoulder itching can arise from a wide variety of reasons. In addition to a skin condition localized to the shoulder, it could also be a neurologic issue. Furthermore, it could also be a mild referred pain related to abdominal problems (like the gallbladder), pelvic issues (like a ruptured cyst), or lung disease (like a pneumonia). As you can see, there are many possibilities.

Hard to say: hard to say without more details! Are there any changes in the skin? Dryness? Redness? Spots or small bumps? Is there a pattern to the rash/itch spot? How long have you had the itch? Is it spreading? Has it been treated in the past? History of eczema? New soap. shampoo or detergent? If it is keeping you up it must be pretty severe and would be worth seeing your doctor/dermatologist!
