EASY, no worries: The Health care provider should ask you questions to be answered honestly (a Medical history-may include sexual history), and then the exam would include a visual inspection of the Genitals (penis, scrotum, skin, groin) as well as a palpable exam of your testicles to check for any abnormalities, and possibly a hernia check. Tests may be run if needed for stds, etc. (usually urine and/or blood).
