Doesn't get in brain: Neostigmine's chemical structure (quaternary amine) does not allow it to cross the blood brain barrier, so it does not affect cholinesterases in the brain.

Would make it worse: Neostigmine is blocking effect of acetylcholine and would probably make the cognitive deficit in alzheimer§s disease worse.
