Is there a prescription with both iodine and iodide in it that doctors prescribe?

Iodine: is easily converted to iodide. Salt has it in the iodate form http://www.ncbi.nlm.nih.gov/pubmed/11396703 Since people are getting it in the iodate form and it converts easily back and forth, not much reason to give it in both forms. The liquid is iodine, the tablet is potassium iodide. Mainly, do NOT take huge doses because it causes heavy metal toxicity (u need a LOT before it is toxic, however)
