See ENT doctor: Fluid in the middle ear without pain is the result of Eustachian tube blockage. At 48 years of age, if you do not have any history of previous otologic disease, then if you are unhappy about the hearing loss, pressure or fullness that frequently accompanies this problem, I would recommend you see an ENT doctor for evaluation. Steroids for 5 days could be tried and if not helpful, then ear tap.
