Thrombosis?: A varicose vein can appear lumpy and grape-like. If it is soft to touch then it is probably a routine varicose vein. If it is hard then it could mean that the blood in the vein clotted . This is called thrombosis. If it is hard and tender, then this is called superficial thrombophlebitis. Regardless of what you may have, you should see a vein specialist for a venous evaluation.

Varicose vein: your describing a lump in a varicose vein which raises the question of is this a blood clot. It could also represent enlargement of varicose vein due to valvular dysfunction. I would recommend a venous duplex ultrasound to differentiate, and decide on possible treatment.
