Call his doctor: You should call his doctor or the local mental health crisis center. If he is a danger to himself or others, his doctor will want to admit him to stabilize him. Be well and be safe.

Is he demented?: Someone who is paranoid for whatever the reason is difficult to reason with and so, "making" him take medicine would not be the best approach. If he's having a good day and you OFFER and he takes, great....if he's having a bad day then, check with your doctor or pharmacist and see if the medicine can be mixed in with food or beverage. Sometimes capsules can be opened and sprinkled on food.
