Inpatient care: It sounds like an intensive inpatient behavioral setting would be best. Horrible recent assault in context of a tragic family of origin with active psychotic symptoms. She needs a therapeutic setting where she is & feels safe & supported.

Dual Services: It really depends on her location and financial standing. Regular psychiatric coverage and weekly trauma based psychotherapy at a minimum, based on their current status, may need inpatient stabilization.

Psychiatrist: it sounds like you are talking about someone other than yourself. Excessive stimulants, cannabis, or phenobarbital could be cause of paranoia and hallucinations. Childhood trauma could result in nightmares an/or flashbacks of traumatic events. Psychiatric consultation is definitely indicated. I would be happy to provide email or virtual consultation, but need to discuss with patient directly
