What are the chances of being pregnant if on birth control, an antibiotic, and psychiatric medicine?

Possible: No birth control is 100% effective besides abstinence. Birth control pills interact with other medications, so without knowing what birth control, antibiotic and psychotropic you're on, it's hard to tell what your chances are.

Nearly zero: Antibiotics don't make birth control inactive like was previously thought. This has been soundly proven now.

Med interactions: Some meds can negatively impact the effectiveness of other meds. For example some antibiotics can decrease the efficacy of oral contraceptives. Which could leave the patient at risk of becoming pregnant.
