Could be muscular : I'm not sure how related sleeping with the ac can be but pain worse with twisting and bending is usually muscular in origin. Having said that- it doesn't mean it any be something else so I would suggest being seen by a doctor.

Sounds muscular: but depending on her age, blood pressure, lipids, and other heart risk factors, it could be any number of things. She should see a doctor.
