Extensive tumor necrosis! how to diagnose, what to do?

Remove if possible: When tumor get very large and essentially loose blood supply necrosis can set in. If extensive, tumor necrosis syndrome develops and release of toxins into the blood stream can occur. At this point debulking of the lesion is beneficial.
