A number of causes: Fever with enlarged spleen and liver (hepatosplenomegaly) in a child can be a variety of things. Many viral infections--Epstein-Barr and CMV in particular--could do this. Some animal-borne bacterial infections or even TB can do it as well. Finally, there are more dangerous things like leukemia and lymphoma that can present this way. Consult with your pediatrician or possibly an ID specialist.
