Mole removed(w/out biopsy) & w/in 2 weeks it grew back. Not perfect circle & dark center & gets lighter until blends w/ skin around.Should i b concerned?

I have had this mole for over 3 years that's always hangs off like a skin tag. it's always been brown, round like a ball, pretty normal. the "stem" does seem to be white, and is the same color of my skin. any concern? not sure if this is new or not?
If a small skin mole is accidentaly scraped off and normal skin or scar remains (no mole re growth ) is that a concern?
