None of above: Neuritic. Your symptoms can be explained by a combination of neuropathies from the cervical roots C4-7 to the brachial plexus to a median nerve entrapment (carpal tunnel) - particularly the intermittent tingling in your 1st, 2nd (index), 3rd, and 4th fingers. A careful neurologic exam should be able to determine the level of the pathology.
