Yes: You need a bit more of an assessment.... The difficulty voiding (sphincter tone-general medical exam) constipation and medication side effects may be contributing to the problem, if I am understanding your question.

Herniation: Lumbar disc herniation per this report can give you difficultiers with going to the BR; often it is pain / discomfort related; only when there is severe obstruction (report doesnt look too concerning...) can there be retention or incontinence if the problem worsens.... hopefully with some non-operative measures, things will improve!
