See answer: I am making assumption that you are referring to the scrotal sac and not the actual testicle. If true, most common cause of what you describe would be an inflamed/infected hair follicle/sebaceous cyst possibly aggravated by attempts at squeezing to express its contents which can lead to a spreading cellulitis or abscess. Good hygiene and local heat may resolve problem. If worsens, needs antibiotic.
