Careful exam by MD: A good physical exam and history can usually determine where the pain is originating from.

Few thoughts: Complete neurologic examination, including sensory, motor tests, reflexes. Emg / nerve conduction velocity study, if questionable results. Sympathetic pain diagnosis, however, is 100% clinical (based on history and examination) always consider temporomandibular joint disorder for facial, neck, and shoulder pain.
