Yes: If you are not at all improved, a call to the doctor is in order. If the medication is working, symptoms usually improve within 24 to48 hours of starting the medication. Was a culture done? If so, this will help guide the therapy. If you have fever, back pain, flu like feeling call the doctor right away. Either way, you should have a follow up visit.

Renal Calculus?: You are being treated for pyelonephritis. Is there a possibility of a renal calculus (kidney stone)? Where is the pain occurring? An abdominal sonogram (a noninvasive test) could rule out kidney stone. This should be evaluated.
