Urology: Severe recureent epididimyttis is a true urological issue. I stongly advise urology consultation and even more aggressive antibiotic therapy. He will also need testicular ultrasounds and should abstain sexually for at least 10 days. Call a urologist-asap.

Chronic: Assure he has been seeing a urologist to assess and definitively diagnose such a chronic infection, as having to be on antibiotics forever sounds unfortunate. Best wishes.
