Other symptoms are ?: The average 5 yo will go thru 5-7 viral respiratory & GI illnesses per year, many of which will generate fever upwards of 102-103.It is a normal response to illness & doesn't signal severity of a problem. If the kid is playful, taking fluids well & has no scary complaints, the best person to evaluate the child is your regular dr. If quiet, refusing fluids, in obvious pain or showing scary sx go.

Not an easy task: Have you discussed this with your doctor and or a behavior therapist? I do not think there is a short answer to this....Understanding how your child sees things my help you find a strategy he/she can accept...Homeopathy and chinese medicine may also be very helpful by looking at the whole picture and finding the root cause.

Not normal: This obviously is not normal. He needs a full evaluation from both physical and behavioral standpoints. For instance, is he hearing ok. If there is nothing physically wrong, one has to entertain the possibility of developmental speech delays, of which there are different causes. A developmental pediatrician and a pediatric speech/language pathologist can help you out there.

Doctor can evaluate: Healthy people can get stomach flu or food poisoning symptoms occasionally. One will usually vomit until his stomach is empty, which is why it is important NOT eat or drink anything for a few hours afterwards. A usual treatment is small amounts of Pedialyte or broth, maybe 0.5-1 ounce every half hour for 1 day during awake hours. If worrisome symptoms like getting weak or pale, Dr. or ER can help.
