Couple of causes: There are a couple of causes for white spots on the tongue. A common one at that age would be bite marks from his incisors. At 21 months, he is old to have thrush, unless he was recently on an antibiotic. Some viral infections may cause similar lesions. If they act like they bother him, are worsening, or have been there for a long time, i would suggest seeing his pediatrician.
