Often not much: An ulcer surrounded by firmness, or just a slight firmness and expansion of an area with a bit of surface roughening, may be all you notice at first. Neglected, it'll turn much nastier in time. Photo Guy let his go for a while, but it looks like his surgeons did a great job with reconstruction.
