Many ddx: The differentials are broad including appendicitis, cholecystitis, bacterial peritonitis, diverticulitis a thorough eval in the ed is indicated.

Multiple issues/labs: Patient must be examined and have comprehensive blood tests, urine tests, ultrasounds and ctscan of the abdomen: differntial is quite extensive and needs more data.Ivs and shock precautions and then ICU care.
