I have been taking tamoxifen since 2011. at first, the hot flashes were intolerable, but they diminished over time. they have now returned with a vengeance. what might account for this?

Hormone effect: I assume the tamoxifen was given 3 years ago prophylactically for a breast malignancy. The purpose is to block estrogen effect on the breast and er+ tomor cells. Here tamoxifen is an antiestrogen. The effect on the uterus is just the oppostie and the drug as an estrogenic drug, causing your symptoms, and endometrial hyperplasia which if present may lead to endometrial ca.
