Try WPATH.org: The referral directory published by the world professional association for transgender health (WPATH) is reliable place to start: http://bit.Ly/1ntyqde Also, search on HealthTap for docs in your city who self-identify as trans* knowledgeable (I found several) & call their offices. As far as a letter, not all docs require one. Current WPATH Standards of Care guidelines are just that - guidelines.
