Asthma attack: I would recommend an emergency type facility... Either hospital er or free -standing emergency center/clinic where you can be monitored more closely as your treated.

Closest place.: As long as both places have adequate medical personnel and the ability to give you a nebulization treatment, you should go to the closer facility. If they are the same distance away, i would recommend the er.

Go to ER again: Your symptoms suggest that you may have a collapsed lung and thus not responding to usual treatment. Go to the er and get yourself examined asap.
