Infection: Probably some kind of infection, despite the lack of visible swelling. Especially likely if you have any history of IV drug use. Or, if your other infections are recent, could be throwing off emboli themselves from the heart. You need to get some blood tests to check for infection right away, since untreated this may complicate your heart issues more.
