Take care: That does not necessarily mean anything grave at present but that depends on how much the septum is hypertrophied compared to the other walls. At any rate it should be followed regularly with echo probably every 12 months and your blood pressure should be kept in very good control at all times.

Will try : Mild concentric ventricular hypertrophy means that the heart muscle is mildly thickened all around. This can be seen in people with longstanding hypertension not yet optimally controlled. Mild left atrial dilation means that the top chamber of the heart on the left side is slightly big. This may be due to problems with the heart valves on the left side of the heart, heart rhythm problems, etc.
