May depend how much: The answer to this depends somewhat on any underlying medical conditions (alcoholism, for example) as well as how much blood is present. Sometimes, when people have been coughing a great deal, for example, small blood vessels in the esophagus can break, making the sputum somewhat bloody. If there is a great deal of blood, though, it is definitely something that should be checked out right away.
