1: Yes, even though most common cause of esophageal spasms is acid reflux spasms can happen unrelated to heartburn. Esophageal dysmotility is an established cause of esophageal spasms and manifest as chest pain and difficulty with swallowing.

Esophagus spasm pain: Painful esophageal spasms are often distinct from classic manifestations of acid reflux. Sufferers of esophageal spasms may experience unpredictable & significant knife-like pain &/or pressure that may radiate to back & chest due to esophageal dysmotilities (diffuse esophageal spasm, nutcracker esophagus), or be sporadic. Treating spasms may exacerbate gerd by relaxing lower esophageal sphincter.
