Swelling. : It's very common for children under the age of 6 to get fluid in the middle ear that causes pressure and head ache. Not an infection. The swelling below the ear needs to be checked for abscess vs lymph node enlargement. If its tender, infection is most likely regardless of fever or none. See your doctor to get this checked as soon as possible.
