It's tricky: It is often difficult to determine who is at the end of life in 3 months vs. who is at the end of life in 3 years. Doctors are usually the ones who make the ultimate determination in conjunction with knowledgeable hospice workers. I've admitted patients to hospice and then a year or so later had to "un-admit" them because the defied my prediction.
