It can be.: Dystonias can be both painful and scary, especially when they first start and are unexpected. They essentially are like a severe muscle cramp that goes on and on.

It depends: Dystonia describes a form of muscle reaction where muscles cramp up or spasm in response to usual actions. It can be very localized to throat or hand or feet or can be very generalized. In severe forms the spasms can be very painful. Mild forms are annoying and frustrating but not painful.
