Diabetes: Having protein, sugar and blood in urine indicate kidney disease. That might need biopsy. Also diabetes causes sugar, protein and ketones in urine.

Worrying: Blood in the urine may require workup for tumor. This is the most worrisome. I'm assuming that "no medications" means you are not taking unregulated "holistic" medicines (may be liver / kidney poisons). Your physician will make the call as to how to work up the rest based on how far off the values are.
