You decide: No one can force you to be treated, or to accept anything beyond comfort measures which may include local radiation for pain. If you are a candidate for palliative chemotherapy, and you have something you want to do before beginning treatment, ask a physician that you trust. Especially if you have small cell carcinoma, you can expect at least several good months after initial treatment. Good luck.

Stage 4 lung Ca. : A few months, somewhere between 3 months or shorter to 8 months on average depending on the extent of disease and tumor aggressiveness. But i want to emphasize stage 4 lung cancer is not curable, but treatable. I encourage you to see local oncologist, fully review the case, and possibly start treatment as soon as possible.
