Variable: They can be totally occluded or have non signicant plaque. The degree of blockage is expressed in percentage so that 80% or greater is considered for intervention when unilateral or when associalted with symptoms such as tias or if the other side is blocked at even lower percentage such as 70%.

0-100%: Different grading systems. Less than 30%, then 30-49%, then 50-69%, then 70-99%, then 99% or tight, then occluded.

Generally: Generally the major trials regarding decision making in carotid stenosis provide guidance at: 1) 70%, and 4) occluded (100%). There are many other grades that have been developed and discussed, but their clinical utility from the vascular surgeon's perspective is limited. Discuss further with a board certified vascular surgeon.
