Observation v repair: Many hydroceles are small and asymptomatic and do not merit any treatment. If the hydrocele comes and goes, it is a "communicating hydrocele" which merits repair. If not, but if it is very large, or symptomatic, then surgical repair is usually performed, although needle drainage with or without sclerosis are used occasionally.
