Symptoms?: Often your symptoms dictate what insurances will cover. Usually they do not pay for screenings but if you have severe headache, blurred vision, dizziness, or any other neurological symptoms, insurance will pay. Your doctor may have to do a doctor to doctor review (peer to peer) and get the scan approved. Ask your md what codes he used initially to have the scan done. It is a game of life.
