Some thots: The EMG suggests presence of peripheral neuropathy, and the additional issues may suggest thyroid, b-12, or other nutritional issues. Am somewhat baffled as to why you seem to lack diagnosis or therapeutic advice. Propose you see a neurologist at a nearby medical school, and get an additional evaluation.

Neurology: You should see a neurologist if you have not, need to rule out something like multiple sclerosis.
