Swelling: If it is only one hand, it could be some kind of trauma, or even an insect bite. If both hands, it could be many things. You should have it checked, by your doctor.

M.D. visit/ice: It may be caused by a number of conditions. I would suggest checking your child's pulse and looking for any discoloration of the skin or nail beds. If there is a puncture site, this may be related to an insect bite or puncture wound from another cause. Ice may help to relieve the swelling and I would suggest follow up with your pediatrician.
