Several things: It takes a medical evaluation to know for sure, and is based on the symptoms the baby is having ( does the baby appear short of breath, coughing up phelgm, fever present etc, findings on physical exam, and possibly the results of lab tests and a chest xray. In any case if a baby looks very ill or is not getting better this suggests a need for further evaluation asap.
