Rarely: My own daughter went through a phase of excessive hand washing at age 3 , and she is now a well adjusted 16 year old, most likely a transient phase.

It is possible: It is possible. Many children that young have some behaviors that might be called ocd...They like to line up their toys etc...It is hard to label it true OCD that lung. If you are concerned about this then being seen by your physician to discuss it further is warranted.
