Yes: Usually by the time a tumor is visible on chest x-ray it is fairly large. There is currently no reliable screening procedure for lung cancer.

Certainly: Sometimes densities less than 5 mm can be not seen due to location and overlying structures. CT scan is much more sensitive for detecting smaller lesions because of thin transaxial slices through the lungs.
