It Doesn't : An electrocardiogram is not used to diagnose high blood pressure. However, sometimes it can indicate possible changes to the heart that can occur with prolonged high blood pressure, such as enlargement of the heart (ventricular hypertrophy).

Thickening of heart: With long standing high blood pressure, the walls of heart thicken. That thickening can be picked up on ekg. It's called left ventricular hypertrophy or lvh.
