Page the doctor: I believe you can get by paging your pcp on call for instructions. Treat your fever aggressively. Your pcp may just phone in antibiotics for you.

Call PCP back: Were you told to wait before you had the fever and worsened cough? With these new symptoms, it is sounding more like it could be pneumonia or flu, which should get checked out. It likely wouldn't need an er but if your pcp can't see you urgent care would be a good next step. Good luck.
