Different procedures: Hair extensions are fake hair pieces from other people or synthetic sources weaved or attached to ur own hair to create length or volume. Hair replacement and transplant are similar terms describing a surgical procedure in which growing hair follicles are placed in an area of the scalp that is balding.

Different: Hair replacement and hair transplants often refer to the same procedure. Hair is taken from the donor scalp from the back of the head, processed, and inserted into the balding area one follicle at a time. Hair extensions is the attachment of hair to your own hair which is temporary.
